Congratulations on choosing midwifery care!

We are excited to join you on this journey!

That being said, there is much to do and read! Please prioritize and allow ample time to take care of all matters regarding your midwifery care.

Preparation
You will be sent via email or via snail mail a packet of items to read and fill out before the first visit. Allow an hour to read through all the details and another hour to fill out the Intake Form.

Everyone: Birth and Well Woman Services
-Obtain any past medical records that are pertinent to your care (i.e. any other prenatal care you have received)
-Keep a food journal for 4-7 days
-Complete the Intake
-Obtain your insurance card.
-You and your partner (if applicable) read and sign the Informed Decision Regarding Midwifery Care, Licensed Midwife Discloser, Arbitration Agreement, Financial Agreement and your Bill of Rights. All paperwork will be completed at the second visit.

Birth Services
-Be preparted to pay a refundable "Addtional Fees" deposit of $300 at the initial visit.
-Be prepared to pay a 25% non-refundable portion by the second visit.
-If you are using a payment plan, determine before the initial visit when you will make each payment. All funds are due by the 36th week of pregnancy.

Well Woman Care
-Be prepared to pay for the services in full.
-Be prepared to pay a refundable "Addtional Fees" deposit of $100.

Risk Assessment
While we are always assessing your risk status with regards to your health, the initial visit is the preliminary opportunity for us to get a sense of your health and will help us determine if you are eligible for midwifery care.
