If your insurance company accepts your claims, you will be reimbursed according to your insurance plan. If your insurance company denies your claims, you are responsible for continuing to pay Mandala Midwifery for the services.
Sliding scale is not available for those who's plans cover midwifery services.
Unless you would like to undertake the billing yourself, you must work through LBS to file the claims. Their fee for Birth Services is 8% of collections from billing, or $100.00 per full-term client, whichever is greater. Their fee for Well Woman Care is 8% of collections from billing, or $15.00, whichever is greater.
